Abstract Background: The referral system is implemented with the aim of providing quality health services, so that service objectives can be achieved well without having to use expensive costs. Health services are carried out in stages, according to medical needs starting from first level health services. Advanced health services can only be provided upon referral from first level health services. Third level health services can only be provided upon referral from second level or first level health services. Midwives and nurses can only make referrals to general practitioners or specialist doctors providing first level health services. The high referral ratio at the Tinanggea Community Health Center exceeds the BPJS health standard by more than 15%. The number of referrals for BPJS participants from 2021 to 2022 has increased by 35%. Objective: The aim of this research is to analyze the implementation of the patient referral system for BPJS Health participants at the Tinanggea Health Center in terms of the availability of health workers, the availability of health facilities and infrastructure and the availability of medicines. Methods: This research method uses descriptive qualitative research methods using data collection techniques in the form of interviews, observation and documentation involving informants, namely the head of the Tinanggea Community Health Center and general practitioners as key informants, pharmacy staff and nurses as supporting informants and accompanied by triangulation informants. Results: The results of the research show that the availability of health workers still does not meet standards because several types of staff are not yet available at the Community Health Center, availability does not meet HR standards, there are still some of the 155 diseases outside the competence of doctors so referrals need to be made, the availability of health facilities and equipment is still incomplete, The availability of medicines is not yet complete and there are still some patients who ask for referrals at their own request with illnesses that can actually still be treated at the Tinanggea Community Health Center, so referrals will not be made except in certain cases and depending on the severity of the disease. Conclusions: The implementation of the patient referral system for BPJS participants at the Tinanggea Community Health Center has not run optimally because there are still not yet available health workers, the health facilities are not yet complete to provide health services so patients have to be referred due to limited health facilities, and the medicines are not yet complete so it is necessary to the health center to meet the standards of health workers, facilities and infrastructure, and medicines so that health services can run well and can reduce the number of referrals
